Schedule overview: Opening shifts can begin as early as 4am. Closing shifts may end as late as 8:30pm. The schedule is made well in advance and shifts are generally 10-12hrs long. Saturdays are rotated, and on Sundays we are closed.

If you haven't considered Nephrology before, read on as we think that you should. Dialysis Experience is NOT required. We provide comprehensive on the job training that will help you become a specialist in this area of nursing.

Some details about this position:

SCHEDULE: You must have a flexible schedule and be able to work opening and closing shifts, rotated Saturdays, and some holidays. We are closed on Sundays! Full-time teammates generally work 3-4 days per week and the shifts are 8-12 hours in length.
LICENSE: Current Registered Nurse (RN) license in the state of practice; Graduate Nurse pre-licensure available in some locations
DEGREE: Associates Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree
TRAINING: Training may take place in a nearby clinic with teammates other than your assigned home clinic.
Flexibility on home clinic assignment - There is the potential to float to nearby clinics.
A desire to deliver care in an empathetic, compassionate way. You know, the way you'd want your own care to be
Experience assessing, trouble shooting and making sound recommendations in stressful situations
Excellent communication skills to listen and communicate with patients and teammates
Basic computer skills and proficiency in MS Word and Outlook. You will be required to chart patient's assessment, orders and your care notes in our electronic medical record system.

Not required, but "nice to haves":

Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience

Description and Requirements

You will work five days, Monday through Friday either mornings or afternoons which includes 4 hour shifts in 2 assigned boroughs. As an Assessment RN (UAS) you will visit members in their homes at intervals required by the Department of Health (DOH) and complete appropriate documentation to determine members' enrollment, service needs, and continued plan eligibility. This position coordinates with Care Management Team (CMT) to maintain efficient and effective operations of Healthfirst's long term care plans such as CompleteCare, FIDA, and Senior Health Partners.

Duties and Responsibilities:

Conducts in-person (and virtual assessments, as needed) for prospective members and re-assessments of current members using the Uniform Assessment System (UAS); make enrollment and continued eligibility determinations by utilizing clinical expertise and critical thinking skills. As needed, conducts assessments in settings other than home, e.g. skilled nursing facilities or virtually
Completes assessment of service needs per CMT and member's family/caregiver's request to determine service needs or any necessary adjustments
Reviews previous nursing assessments/medical notes to determine necessary adjustments/updates in Care Plan and/or to assist with nursing home admission
Conducts in-person Care Management visits in support of person-centered service planning
Liaisons between the member and the CMT; assesses home environment and psychosocial status
Provides feedback to home care agencies and CMT regarding home care issues and contract nurses and aides performance
Provides grass roots, community-based training for frail population care including self-care techniques and prevention strategies
Ensures (prospective) members and significant others/responsible parties understand and agree with enrollment in a managed long-term care plan
Fulfills all scheduling activities to ensure prospects/ members/ caregivers are scheduled and are aware of the scheduled visits
Be able to fully participate in orientation, training and meeting activities as the role requires
Additional duties as assigned

Minimum Qualifications:

New York State RN license
At least 1 year of prior UAS Assessment Nurse experience
Ability to travel within the Bronx

Preferred Qualifications:

Previous field-based experience assessing, planning, and evaluating member's care by making home or facility visits for intake/reassessments or start of care (SOC) Community Home Health Agency (CHHA) visits
Work experience preferred in one or more of the following areas, geriatrics, home care, discharge planning, case management, and/or medical surgical nursing
Work experience using electronic patient health information (PHI) database usage especially UAS
Unexpired NY state driver's license or NYS ID
Knowledge of health insurance, Medicaid, Medicare and MLTCP
Experience working with a frail adult or elderly population with the ability to determine appropriate care plans and services for frail population as well as negotiate initial service plans so that members and families agree
Time management, critical/creative thinking, communication, and problem-solving skills
Language preferences - Spanish, Russian, French, Creole, Mandarin, Cantonese
Demonstrated ability handling heavy caseloads
Intermediate Microsoft Word, Excel, and Outlook skills

Hiring Range:

Greater New York City Area (NY, NJ, CT residents): $81,099 - $116,480
All Other Locations (within approved locations): $71,594 - $106,080
